{"id":17708,"date":"2018-10-09T04:24:04","date_gmt":"2018-10-09T04:24:04","guid":{"rendered":"https:\/\/qatar-news.org\/qatarnewsEn\/?p=17708"},"modified":"2018-10-09T04:24:04","modified_gmt":"2018-10-09T04:24:04","slug":"saving-the-traditional-midwife-in-guatemala","status":"publish","type":"post","link":"https:\/\/qatar-news.org\/qatarnewsEn\/?p=17708","title":{"rendered":"Saving the traditional midwife in Guatemala"},"content":{"rendered":"<div id=\"body-140415133408172\" readability=\"186.130681818\">\n<p dir=\"ltr\"><strong>Chiquirichapa, Guatemala-<\/strong> Antonina Sanchez Mendez vividly remembers the first time she delivered a baby.<\/p>\n<p dir=\"ltr\">The only tool she had was a razor to cut the umbilical cord.<strong>\u00a0<\/strong><\/p>\n<p dir=\"ltr\">&#8220;I didn&#8217;t know what to do,&#8221; she tells Al Jazeera.<\/p>\n<p dir=\"ltr\">&#8220;When the baby was born, I was trembling and sweating,&#8221; she recalls.<\/p>\n<p dir=\"ltr\">Although she wanted to keep the experience private, the mother &#8211; a friend of Mendez &#8211; whose baby had just been delivered told others in the small Guatemalan town of Concepcion Chiquirichapa\u00a0about her experience, prompting pregnant women to begin seeking out Mendez for help.<\/p>\n<p dir=\"ltr\">That was more than 19 years ago. Since then, Mendez has helped deliver hundreds of babies, taking on a vital role in her community where there&#8217;s a shortage of traditional indigenous midwives.<\/p>\n<h2 dir=\"ltr\">High mortality rates\u00a0<\/h2>\n<p dir=\"ltr\">In Guatemala, indigenous women are <a href=\"https:\/\/www.dfcm.utoronto.ca\/event\/maternal-newborn-and-child-health-mnch-guatemala-transforming-indigenous-lives\" target=\"_blank\">twice<\/a> as likely to die during pregnancy or childbirth as non-Indigenous women, and Indigenous infants are two-thirds more likely to die than non-Indigenous infants.\u00a0<\/p>\n<p dir=\"ltr\">This is compounded by the fact that indigenous Mayan women are less inclined to go to the country&#8217;s Ministry of Health facilities due to language barriers, the lack of understanding of indigenous traditions and the expense.<\/p>\n<p dir=\"ltr\">An appointment with a midwife costs about 10 Quetzales ($1.40), but a consultation with a doctor between 50 and 100 Quetzales ($7 and $14) per visit.<\/p>\n<p dir=\"ltr\">Such challenges, as well as the value found in traditional medicine, have made midwifery within indigenous communities like Mendez&#8217;s even more important, but a strained relationship with the government over the years has contributed to the shrinking network of indigenous midwives.<\/p>\n<div>\n<div class=\"imagecontainer item\" data-image-url=\"http:\/\/www.aljazeera.com\/mritems\/Images\/2018\/9\/23\/ceb76bffb49e43a6b2cf8f2ba70d1417_18.jpg\">\n<table class=\"image\" border=\"0\">\n<tbody readability=\"2.5\">\n<tr>\n<td><img decoding=\"async\" src=\"http:\/\/www.aljazeera.com\/mritems\/Images\/2018\/9\/23\/ceb76bffb49e43a6b2cf8f2ba70d1417_18.jpg\" border=\"0\"><\/td>\n<\/tr>\n<tr readability=\"7.5\">\n<td class=\"caption\" readability=\"8\">\n<p>According to the World Health Organization, the maternal mortality rate in indigenous communities is\u00a0 200 to 300 women per 100,000 live births in Guatemala [Ali Rae\/Al Jazeera]<\/p>\n<\/td>\n<\/tr>\n<\/tbody>\n<\/table>\n<\/div>\n<\/div>\n<h2 dir=\"ltr\">Complicated relationship\u00a0<\/h2>\n<p dir=\"ltr\">A government-run training programme for midwives was started in 1969 as a result of the creation of the Division of Maternal and Child Health.<\/p>\n<p dir=\"ltr\">Since the training programme began, only midwives who attended it and received a certificate were authorised by the government to help with deliveries and register newborns.<\/p>\n<p dir=\"ltr\">In some municipalities, the training programme is taught in Mayan languages. In others, however, animosity towards the traditional practice prevails.<\/p>\n<p dir=\"ltr\">&#8220;The government is trying to make an effort to integrate both visions, but it has proven to be difficult,&#8221; says Oscar Barreneche, a representative with the World Health Organization in Guatemala.<\/p>\n<p dir=\"ltr\">Despite the improvements, many obstacles remain.<\/p>\n<p dir=\"ltr\">&#8220;There is still a very patronising attitude towards these healers,&#8221; he says.<\/p>\n<p dir=\"ltr\">&#8220;They are not always included in all the efforts, and the value of her practices is not always recognised.&#8221;<\/p>\n<p dir=\"ltr\">Midwives, including Mendez, also feel this exclusion when trying to do their work.<\/p>\n<p dir=\"ltr\">&#8220;In hospitals, they believe midwives can&#8217;t take care of births because we don&#8217;t have a degree,&#8221; Mendez says.<\/p>\n<p dir=\"ltr\">&#8220;They believe we don&#8217;t know how to do our work, but thank God &#8211; because of the training we have received &#8211; we know what births we can take care of and which ones we can&#8217;t.&#8221;<\/p>\n<div>\n<div class=\"imagecontainer item\" data-image-url=\"http:\/\/www.aljazeera.com\/mritems\/Images\/2018\/9\/23\/82d9dcadc0e644da830c823c2597d8c6_18.jpg\">\n<table class=\"image\" border=\"0\">\n<tbody readability=\"1.5\">\n<tr>\n<td><img decoding=\"async\" src=\"http:\/\/www.aljazeera.com\/mritems\/Images\/2018\/9\/23\/82d9dcadc0e644da830c823c2597d8c6_18.jpg\" border=\"0\"><\/td>\n<\/tr>\n<tr readability=\"4.5\">\n<td class=\"caption\" readability=\"6\">\n<p>The expense and lack of access to modern healthcare have made midwifery<span>\u00a0even more important<\/span> within indigenous communities [Ali Rae\/Al Jazeera]<\/p>\n<\/td>\n<\/tr>\n<\/tbody>\n<\/table>\n<\/div>\n<\/div>\n<h2 dir=\"ltr\">Rescuing the tradition \u00a0<\/h2>\n<p dir=\"ltr\">Although Mendez says this has contributed to a shortage in the number of women who want to become midwives in her own community, she remains dedicated to keeping the ancient tradition of midwifery alive.<\/p>\n<p dir=\"ltr\">&#8220;If the midwives died without training the new generation, the culture dies with them. There are a lot of traditions involved,&#8221; she explains.\u00a0<\/p>\n<p dir=\"ltr\"><span id=\"docs-internal-guid-89a321ff-7fff-8e76-70e3-8e1b5984c9d7\"><span>Mendez receives women at all hours of the day and night. Many arrive for prenatal consultations, or to find relief for pain. Sometimes they come for abdominal massages.<\/span> <\/span><\/p>\n<p dir=\"ltr\">She and others in Concepcion Chiquirichapa, which is about\u00a0<span>214km west of\u00a0<\/span>Guatemala<span>\u00a0City,<\/span> formed an organisation of midwives to strengthen their efforts and educate future generations.<\/p>\n<p dir=\"ltr\">The group built the first Mayan birth centre with two temazcales (a traditional steam bath), a natural medicine laboratory, and eight delivery rooms.
